What is breast preservation?

Traditional breast augmentation involves creating a pocket for an implant, which requires some degree of surgical dissection and disruption of the breast’s natural tissues.

The concept behind breast preservation is different. It aims to create space for the implant while preserving important supporting structures of the breast, including the ligaments that help define its natural boundaries and shape.

One of these is the circummammary ligament, which contributes to the breast’s attachment to the chest wall and helps define the natural breast footprint, including the inframammary fold.

Rather than extensively dividing these supporting tissues to accommodate an implant, tissue-preserving techniques aim to respect and maintain them wherever possible, positioning the implant within the breast’s existing anatomical boundaries.

Why does this matter?

Because an implant shouldn’t be considered in isolation. The tissues surrounding and supporting it are an important part of the final result.

This represents an important evolution in breast augmentation: moving away from simply creating a space large enough for an implant and towards working with the woman’s existing anatomy.

Why is subglandular breast implant placement more popular now?

Before modern, cohesive silicone implants were available, submuscular placement (under the pectoralis chest muscle) was considered the best approach, because it helped to hide visible implant rippling and carried lower rates of capsular contracture than over-the-muscle “subglandular” (directly under the breast gland) implant position.

Although submuscular placement effectively conceals more of the implant, it carries a few potential drawbacks: it requires a longer recovery, and the implant shape can be affected when chest muscles are activated (leading to “animation” of the implant). A very tight, well developed muscle can also pull the implant into a higher position for a longer period of time. This is especially true if a patient is heavily hand-dominant on one side.

For these reasons, many patients are now considering over-the-muscle breast implant placement.

Who is the right candidate for Preservé™?

Preservé is designed for patients seeking a subtle, natural enhancement in the range of one to two cup sizes. The system uses Motiva SmoothSilk Ergonomix2® implant, which accommodate up to 315 cc of volume increase. If you are hoping for a more significant size change, a different implant or technique may be better suited to your goals. Women with good skin elasticity and adequate existing breast tissue often see the best results.
